Establish what happened, in order. The factual timeline separates what the record proves from what people assumed, with work as imagined, work as normal and work as done as their own lanes.
Code the contributing factors properly. Every factor carries a real ICAM code and sits in its family: DF for absent or failed defences, IT for individual and team actions, TE for task and environmental conditions with HF at the human performance level, and OFT for the organisational factor types.
Tie each recommendation to what was found. A recommendation is linked to the factors it answers, so a reviewer can see which findings are addressed and which are not.
The report is assembled from the record: the executive summary, the terms of reference, the severity classification and a definitions table written for readers outside the safety function. The investigator edits and approves.
