ICAM, the Incident Cause Analysis Method, treats an incident as the product of a network of contributing factors rather than a single root cause. SafetyPulse is built on that premise.
Contributing factors are classified into the ICAM families: absent or failed defences, individual and team actions, task and environmental conditions including human factors, and organisational factors. Systemic corrections land at the defence and organisational layers, which is where the method says durable fixes belong.
SafetyPulse is made by ICAM Australia, which teaches the method and has trained more than 7,500 investigators. The software and the training use the same vocabulary and the same factor codes.
