TRIFR tells you what already happened. Everyone knows it, everyone still reports it, and it says almost nothing about next quarter. Yet sitting inside work most organisations have already done is a leading indicator they never read: the quality of their investigations and what happens after them.
The logic is simple. An investigation is your organisation's formal act of learning from an unwanted event. If that learning is shallow, late or unactioned, the conditions that produced the event are still in place, and the next one is already loaded. Investigation records are therefore a forecast, not just a filing obligation. Three questions turn them into one.
Pull your last five reports and classify where the contributing factors landed across the ICAM families. Findings that cluster on Individual/Team Actions (IT), the worker, the shortcut, the missed step, with little depth in Absent/Failed Defences (DF) or Organisational Factor Types (OFT) are a signal in themselves. Not that your people keep failing, but that your investigations stop at the person and the procedure. Shallow findings are upstream of repeat events, because the system that set the person up remains untouched.
Action discipline is one of the most honest signals of safety culture available. Actions that close on time, with verification evidence, at a meaningful level of the hierarchy of controls, tell you the organisation converts findings into change. Actions that quietly roll over month after month, or close administratively the week before an audit, tell you the learning loop is decorative. Neither shows up in TRIFR until it shows up as an incident.
A barrier that keeps appearing in unrelated reports (isolation verification, change management, supervision coverage) is telling you where the next serious event is coming from. Read one investigation and it's a finding; read across ten and it's a pattern with a probability attached. Most organisations never do the second read, because the reports live as PDFs in a folder and nothing connects them.
